If your insurance is not one listed above and you would like to become our patient, please call our office for price quotes (603) 627-6381.

If we do not currently contract with your insurance company, we will provide you with the necessary information for you to submit a claim for reimbursement from your plan.

Do you accept my insurance plan?

If you are unsure, please refer to your insurance plan directory, website or call your insurance company to determine if we are "in network" or "out of network" with your insurance plan.

Do I need a referral for physical therapy or chiropractic care?

Many PPOs, HMOs, and EPOs require a referral from your primary care physician for physical therapy and occasionally for chiropractic. Please check with your insurance plan if you are unsure.

Whose responsibility is it to get my referral authorization?

It is the patient's responsibility. As a courtesy, we will try to assist you in obtaining referrals.

As a patient, do I have any responsibilities when my insurance company does not process or is slow in processing claims?

If medical claims are not paid within 30 days from billing, it is the patient's responsibility to help follow up with the insurance company.

If my insurance company does not pay, am I liable for the bill?

Yes.

Will you bill my secondary insurance after my primary carrier has paid their portion?

Yes, as a courtesy we will bill the secondary insurance.

Will I be seen if I haven't paid my co-pay?

No. Our policy requires payment of co-payments at the time of service.

When is my deductible due?

Your deductible balance will be brought to your attention at check-in. Payment is expected then. You will also receive a mailed statement of any unpaid charges.

Do you accept third party liability insurance (automobile or personal injury)?

No. These cases are set up as self-pay accounts. As a courtesy we will bill your insurance, but require payment at the time of service.

If the parents of a minor (child) patient are divorced, which parent is financially responsible for the child's medical bills?

We consider the adult person who authorized the care to be the responsible party/guarantor.

What do I have to send with my payment?
